VBS DISMISSAL REQUEST FORM

To:  Parents of Pre-school and Elementary aged Students through Grade 5

From:  Christian Education Committee

In our continuing efforts to keep children safe, the Christian Education Committee requests that each child’s parent(s) complete the form below concerning their child’s dismissal from Vacation Bible School. Please include all names of those who may be picking up your children during this week.

Please complete and sign the form and return it to the Director of Christian Education, or a VBS Director.  We have not had any incidents concerning dismissal of children, but we certainly wish to avoid any unfortunate occurrences.
